Context and purpose of the consultation

Guinea Bissau’s national health development plan makes the reduction of maternal and neonatal mortality one of the priority objectives of its sectoral health policy. This approach is aligned with the’Agenda 2030 which advocates universal access to quality maternal and neonatal health care with a view to reducing the maternal mortality ratio to less than 70 maternal deaths per hundred thousand live births. According to INASA 2018, the maternal mortality ratio in Guinea Bissau has been estimated at 746 maternal deaths per 100,000 live births with large disparities between regions. For example, the ratio in the Quinara region was estimated at 3,015/100,000, four times the national average.This alarming figure could be explained, among other things, by the ABSENCE of a substantial technical platform in the region for the management of obstetric activities.

 

In view of the situation, since 2021,’UNFPA has invested in a pilot experiment that has led to i) the rehabilitation and equipment of the health center and operating room, ii) the recruitment of the three specialists an obstetrician doctor/gynecologist surgeon, an anaesthesia-resuscitation technician and an instrumental midwife, and and iii) the provision of reproductive health products, including products that save human life. Building on the encouraging results achieved over 3 years,’UNFPA and the Ministry of Health, wish to deepen the collaboration and propose to strengthen the achievements at the CS of Buba and to organize the transition to’ scale in two other regions with the highest maternal mortality rates in the countrythe low contraceptive prevalence rates as well as the lowest gender and human rights statistics namely, Gabu and Bafata with respectively 704 and 536 maternal deaths per 100 000 live births (INASA 2022).

 

The’ objective of the transition to the’ scale is to contribute to a 50% reduction in maternal mortality in the SONUCs of the three regions covered by the Project (Bafata, Gabu and Quinara) by the end of December 2025. The 18-month project aims tostrengthen the institutional and technical capacities of reference health centres to offer quality services and care in SONU, SRMNI, and family planning; It also aims to strengthen the quality of services at SONUB and SONUC level through the provision of integrated services for the health of the mother and newborn

 

L’UNFPA is looking for a consultant(e) international(e) to support the clinical part of the transition to’, a SONU network in the 3 regions (Bafat, Gabu and Quinara), capacity building of SONU-C to ensure their full functionality.

 

Scope of work (Description of services, activities and products)

The mission of/the consultant (e)international will focus on 4 main axes:

1. Analyze the’offers existing NISRS services for improvement (quality and availability of H24 care, adequate structures, available equipment, available human resources) ;
2. Supervise and support the local technical teams of the 3 SONU-C in their technical tasks and transfer of skills to the SONU-B.
3. Organize the’training offer and provide ongoing mentorship of health providers in the three regions with SRMNI themes.
4. Structure the mechanisms of data collection and’analysis for the purposes of monitoring and’evaluation.

 

Specific objectives :

• Evaluate the existing SR service offer in each of the regions in order to identify the gaps in terms of personal, d’infrastructures, equipment ;
• Accompany the reorganization of maternity services and operating theatres in the target regions with a view to improving the offer of NISRS services ;
• Accompany the follow-up of the staffing of maternity hospitals, operating theatres, laboratories of the three target regions, by ambulances, materials, medicines that save life, kits of’urgences, kits of cesarean section in order to reduce the delays of taking care of the emergencies ;
• Put on/strengthen the reference system and against reference effective and efficient in each region;
• Accompany the implementation and blood banks in each region and the availability of blood products, reagents and tests necessary for their functioning;
• Ensure an accompaniment medical teams deployed by MINSAP and DRS in the SONU-C and SONU-B by strengthening and supervising their technical and surgical skills management of obstetric and gynecological emergencies, as well as the management of dystocic deliveries ;
• Organize periodic training in SRMNI theme to the health providers of the target regions, including for community health workers ;
• Ensure an data collection and analysis systematic and reliable for better monitoring of NISRS indicators ;
• Discuss periodically with the regional health authorities, the Ministry of Health,’UNFPA the difficulties encountered in order to find suitable solutions.

 

Monitoring and control of progress, including reporting requirements

The deliverables below are expected from/the consultant (e) :

• A’ evaluation/analysis report of the existing SRMNI service offer with recommendations and a’ action plan for the reorganization and capacity building of maternal health centre services, and the establishment/strengthening of the SONU network in the 3 regions (1 month after the start of the consultation) ;
• The implementation of’an accompanying plan (mentorship) and training of medical teams deployed in the SONU-C and SONU-B supported by the project ;
• The effective functioning of the review committees for maternal deaths in the 3 project regions ;
• The effective and functional implementation of the mechanisms for the collection and analysis of data on SRMNI indicators and the functioning of SONUs in the regions.
• Monthly, quarterly and annual illustrated reports documenting progress made, difficulties encountered and indicating necessary adjustments ;
• Final report of the end of the mission.

 

All reports will be submitted to’UNFPA and MINSAP according to the periodicity defined in accordance with the procedures of’UNFPA and will be transmitted in electronic version. Photos will be taken to document the mission by the communications officer of the office.

 

Supervisory arrangements

Le/la consultant (e) travaillera sous la supervision du responsable de l’unité Santé Reproductive et en étroite collaboration avec le coordinateur technique du projet et les personnels de l’UNFPA et du gouvernement affectés au projet.

 

Durée de la mission

La mission est prévue pour une durée de 11 mois, renouvelable jusqu’à la fin du projet en décembre 2025

 

Lieu ou les services seront délivrés

Le/la consultant (e) sera basé (e) à Bissau avec des missions régulières et fréquentes dans les régions de Quinara, Bafata et Gabu.

 

Echéancier de paiement

Les honoraires seront payés mensuellement et seront versés une fois que le superviseur du contrat aura certifié l’achèvement satisfaisant et la qualité des services fournis. Les honoraires seront calculés sur une base fixe. Les paiements et tout déplacement connexe seront effectués conformément aux règles et procédures de l’UNFPA.

 

Expertise, qualifications et compétences requises

Pour mener à bien cette mission, le/la consultant (e) devra être titulaire d’un diplôme en médecine avec spécialisation en gynécologie-obstétrique

 

Elle/Il doit, en outre, justifier :

• Minimum de 10 ans d’expérience dans la pratique clinique et la gestion des urgences obstétricales ;
• Expérience avérée dans le fonctionnement de SONU-C et l’accompagnement de la réorganisation et renforcement de services en matière de SRMNI ;
• Expérience avérée de travail sur le terrain d’au moins 5 années, dont une partie en Guinée Bissau ;
• D’une connaissance des technologies et des équipements pour la collecte et l’analyse des données SRMNI ;
• Une expérience avérée de leader et de travail d’équipe dans un environnement multiculturel complexe est souhaitable ;
• De très bonnes capacités de rédaction de rapports techniques ;

 

Compétences linguistiques : Capacité à lire et écrire (rédiger des rapports) en français ou en anglais et à parler en Portugais ou en Créole bissau-guinéen.

 

Communication: excellentes compétences analytiques et rédactionnelles, et capacité à rédiger/éditer une variété de rapports écrits, à faire des commentaires pertinents et à contribuer à partir de diverses sources en temps opportun.

 

Travail d’équipe : bonnes compétences interpersonnelles et capacité à établir et à maintenir des partenariats et des relations de travail efficaces dans un environnement multiculturel et multiethnique, avec tact et dans le respect de la diversité.
