CARE – INNOVATE – TOGETHER:

Since its creation in 2009, ALIMA has reached more than 18 million beneficiaries in 15 countries, mainly in Africa. In 2024, we developed 59 humanitarian medical response projects to meet the needs of populations affected by conflicts, epidemics and extreme poverty. All of these projects support national health authorities through health structures (including hospitals and health centers). Our action results from constant adaptation to realities on the ground and advances in medicine. In 2024, ALIMA reached 5.9 million beneficiaries. We work in partnership, particularly with local NGOs, whenever possible to ensure that our patients benefit from expertise where it is found, whether in their country or in the rest of the world.Furthermore, to improve the humanitarian response, we carry out operational and clinical research projects, particularly in the field of the fight against malnutrition and viral hemorrhagic fevers.

ALIMA TEAM: more than 2000 people currently work for ALIMA. The field teams, as close as possible to patients, receive their support from coordination teams generally based in the capital of the countries of intervention. DESCRIPTION OF THE ALIMA MISSION IN CAR

Since its creation in 2009, ALIMA has led humanitarian interventions focused on health. Present in the Central African Republic since 2013, the organization develops activities focused on the medical-nutritional care of children, reproductive health, primary and secondary health care, response to epidemics as well as operational research. With its 15 years of experience in humanitarian medical action, including in the CAR, the organization relies on both the expertise of international professionals and that of local staff to effectively respond to the needs of populations.

ALIMA benefits from solid experience in strengthening the RCA healthcare offering, both at primary and secondary level. Concerning secondary care, it supports several hospitals in the localities of Obo, Zémio, Bimbo, Boda, Bocaranga and Ngaoundaye. The Obo and Zémio hospitals have been supported by ALIMA since 2019 as part of a medical assistance project intended for host, displaced and refugee populations in Haut-Mbomou. These two structures, fully operational to date, have benefited from financing support from the BEKOU/RELSUDE Fund since 2018. ALIMA strengthened the capacities of the operating room by carrying out rehabilitations and assigning a surgeon as well as an anesthesiologist to ensure emergency managementthus contributing to the transfer of skills at the local level. This position is a continuation of the achievements of the previous phase, with the aim of compensating for the lack of qualified human resources and strengthening the capacities of existing staff of the Ministry of Health at the Zémio secondary hospital, in the Haut-Mbomou prefecture.

In Zemio, ALIMA supports the Ministry of Health through three funding courses including:

  1. FH Project: Contribute to providing emergency medical and nutritional assistance for displaced populations and hosts in the Zémio health zone of the Haut Mbomou Health District in the Central African Republic. (02/01/2026 to 01/31/2026).
  2. Project for <<Access to medical consultations via telemedicine in a context of deteriorated security>>. This project aims to contribute to reducing the morbidity and mortality rate in the insecure Mboki area in Haut Mbomou. It is funded by Grand Challenge Canada for a period of one year (May – December 2026)
  3. A << Medical-nutritional emergency response project for the assistance of displaced people and hosts affected by the crisis in the sub-prefecture of Zémio, Haut-Mbomou Health District with CDCS funding (Ministry of Europe and foreign ministries) for a period of one year (August 2026-May 2027).

Context of Haut Mbomou:

The Haut Mbomou prefecture, located in the southeast of the country, has a population of around 89,026 inhabitants in 2026.

Since 2023, the area has experienced degraded security: clashes between militias, national forces (FACA) and armed groups such as the UPC. This violence causes murders, kidnappings, destruction of property and massive displacements.

According to IPC projections (August 2026), more than 39% of the prefecture’s population lives in severe food insecurity phase 3 and 5% in phase 4, particularly in the sub‑prefectures of Bambouti, Djema and Obo classified in phase 4 (emergency). The internally displaced population (IDPs) numbers in the thousands, and access to livelihoods is hampered by insecurity (blocking roads, fields), making access to health services extremely limited.

The health sector is also affected: less than half of the infrastructures are fully operational, there is less than one doctor per 10,000 inhabitants, and many structures have neither basic equipment (electricity, imaging, reagents) nor essential medicines.

Actors, NGOs and local government are working to improve humanitarian access (rehabilitation of bridges, patrols, multi-sectoral assistance), but access remains difficult and coverage varies depending on the area.

The Zémio sub-prefecture is going through a complex and deep crisis, marked by a continued deterioration in security. The area is affected daily by serious violence, including armed clashes, kidnappings, extrajudicial executions as well as strong intercommunity tensions. These recent attacks have also forced part of the population to flee, causing massive displacements towards the Democratic Republic of Congo (DRC).

On a strictly humanitarian and health level, the situation is seriously deteriorating due to severely limited access and a virtual absence of humanitarian actors on the ground. The local health system is particularly affected: medical structures are under-equipped and, for some, are simply no longer functional, which extremely complicates the care of vulnerable populations remaining there.
