GENERAL CONTEXT

Médecins Sans Frontières is an independent international medico-humanitarian organization which offers assistance to populations in distress, to victims of natural or man-made disasters and to victims of armed conflicts, without discrimination and without distinction of race, religion, creed or political affiliation.

The MSF movement is structured around five operational directorates supported by the 21 sections, 24 associations and other MSF offices around the world. MSF OCBA is one of these directorates. Operations are implemented by field teams and mission coordination teams, as well as organizational units based in Barcelona, ​​Athens and decentralized in Nairobi, Dakar and Amman. Field operations are guided and supported by 5 operational cells, the emergency unit and other departments supporting the operations, including the medical department, the logistics department and the projects and IT office.

MAIN PURPOSE

Organize and perform anesthesia operations in accordance with MSF rules, protocols and universal hygiene standards , in close collaboration with surgeons and medical staff, and strengthen the knowledge and practical skills of anesthetist staff, through training on the heap and the transfer of knowledge, with a view to ensuring the quality of care provided to patients. Ensure the support of nurse anesthetists in the management of post-anaesthetic complications and the implementation of standard general principles of anesthesia and resuscitation

  • Ensure pre-operative consultations in order to assess the operative risks in collaboration with the nurse anesthetist, the surgeon, provide the appropriate information to the patient and decide on the type of anesthetic to be used, in order to guarantee optimal conditions prior to the operation. intervention. Perform all surgical anesthesia in a safe and appropriate manner.
  • Ensure proper preparation of anesthetics, materials and equipment required beforehand and train teams of nurse anesthetists on the efficient management of drugs and materials.
  • Administer anesthetics and maintain anesthesia by respecting the rules of hygiene and asepsis in force. Also ensure that universal aseptic precautions are maintained in the operating room and PCI measures are in place.
  • Accompany and monitor the patient’s parameters throughout the intervention and ensure that the nurse anesthetists are sufficiently equipped to manage any per and post operative complication related to anesthesia.
  • With the teams of nurse anesthetists and the other members working in the operating room, ensure the proper transfer of patients to the recovery room and their return to a complete state of consciousness, and define the analgesic protocol.
  • Ensure that basic general resuscitation measures are put in place for critically ill patients and that related training is organized for doctors and other members for good continuity of care.
  • In conjunction with the surgeon, advise on appropriate postoperative care, including intensive care, including fluid management, oxygenation monitoring, airway management, and physiotherapy and mobilization of the torso, with special attention to pain relief during the waking process.
  • Implement and supervise hygiene rules and procedures as well as MSF protocols in the operating and recovery room and concerning anesthesia equipment (decontamination, sterilization, storage conditions, etc.) in order to ensure safety and the protection of patients and staff.
  • Support the nurse anesthetists in the operating room in the supervision of the operating room pharmacy (orders, inventories, expired products) and ensure the keeping of the operating room register and the monitoring of the consumption and management of empty narcotic vials, in order to maintain an adequate stock.
  • Ensure the proper functioning and maintenance of equipment in the operating room (oxygen extractor, ventilation, etc.).
  • Organize the tasks of the staff under his responsibility and participate in other team management activities (recruitment, training, performance evaluation, motivation, internal/external communication, etc.) in order to guarantee appropriate resources (staff, skills and aptitudes ).
  • Collaborate with the other departments of the project (respond to emergency requests, intervene to support other services including emergencies, intensive care, post-operative, postpartum and delivery rooms, etc.) and participate in any emergency activity or exploratory mission in order to support the MSF project globally through its expertise and skills.
  • Ensure compliance with administrative procedures (complete patient records, forms, statistics, databases, etc.) and develop regular reports covering results, procedures and problematic situations and cases, in order to have correct and up-to-date information regarding daily activity and support decision-making.

MSF Section Specific Responsibilities / Context

  • Identify the technical difficulties related to the practice of anesthesia and the management of postoperative patients in order to provide solutions in order to reduce morbidity and mortality.
  • Remind nurse anesthetists of the different protocols and possible alternatives.
  • Monitor compliance with and application of the various protocols in the operating room, recovery room and maternity ward.
  • Ensure that resuscitation equipment is available in the intensive care departments (CRENI, PEDIATRIC, NEONATOLOGY and MATERNITY).
  • Ensure that resuscitation drugs are available in each emergency kit in the various departments.
  • Management, medical care and transversal follow-up of patients hospitalized in intensive care, in compliance with MSF protocols.
  • Participates in the multi-daily visit of intensive care patients (paediatrics, creni, neonatology and maternity).
  • Management of unstable or serious patients in the emergency room or in other departments.
  • Work in close collaboration with emergencies, the operating room and hospitalization services.
  • Provide training on cardiovascular resuscitation, shock, rehydration in newborns, children and adults.
  • Technical support of the resuscitation to the teams
  • Follow the sterilizers for the good constitution of the resuscitation kit
  • Build a complete resuscitation kit for emergencies and intensive care (paediatrics and maternity)
  • Set up a resuscitation protocol (heart failure, renal failure, hypovolemic shock, etc.).
  • Proposal and recommendation in its specific areas of expertise: antibiotic therapy, hemodynamics, etc.
  • In collaboration with the various activity managers and in his/her fields of expertise, he/she provides supervision and continuous training for the team (medical and paramedical) practicing intensive care, emergencies, recovery room or in hospital. For this, it relies on the protocols and educational resources validated by MSF.
  • Support the head of infection prevention and control and hygienists in the operating room in the organization of work while ensuring that universal precautions are always respected and contribute to reducing biological risks and improving infection control by putting special emphasis on preventive measures against covid-19.
  • Work in collaboration with MSF and MoH doctors in the various departments in order to maintain the climate of collaboration and ensure the gradual transfer of skills.
  • Organize and participate in death analyzes for all departments and ensure that an action plan and follow-up of recommendations is in place.
  • Discuss with the medical profession the orientations to be taken in the management of complicated cases as well as any obstacle that may impact the delivery of quality care.
  • Inform his direct supervisor or the MAM/Gynecologists in time of any problem that may interfere with the conduct of activities in the hospital.

In collaboration with the NAM/MAM, ensure the quality of storage and availability of drugs in the operating room. while setting up a monitoring and control system.

PROFESSIONAL SKILLS

  • Essential – State-recognized anaesthetist-resuscitator diploma.
  • Essential – Level B2 French. English desired.
  • Desired – Two years of experience as an anesthesiologist-resuscitator.
  • Desired – Experience working with MSF or other NGOs in developing countries.
  • Desired – Experience in the Sahel.
  • Essential – Proficiency in office tools (Word, Excel, Power point, Internet, etc.).
