Community Health Supervisor (Bucaramanga, Barranquilla) At Medical Teams International



Role Objective:

The Community Health Supervisor will be in charge of supervising all the community activities of the program, and managing the pre-selection of the target population, facilitating a referral pathway to accredited medical/health facilities, and is also responsible for Coordinate the volunteer program. The position is flexible and adapts to the needs of the population.

The Community Health Supervisor coordinates strong relationships with the volunteer team, community, community leaders, and other local health stakeholders. She identifies health needs, especially pregnant women, women of childbearing age, mental health, people and families with symptoms of COVID 19, sick children under 5 years of age.

Essential Duties:

Training and information sharing at the community level

  • Manage, together with the Community Health Coordinator, the rooms, the calendar and logistics (assembly, security, provision of visibility elements and personal protection) in the Health Points and other activities in the community.
  • Develop creative community health proposals focused on the target population, taking into account specific needs.
  • Design and implement training and information processes on basic public health messages aimed at populations and volunteers, especially pregnant women, women of reproductive age, and families with sick children under 5 years of age.
  • Supervise the selection criteria made by the volunteers for the health evaluations that are carried out, to the migrants in order to refer them to the Health establishments.
  • Coordinate with community health volunteers for monitoring and regular home visits.
  • Supervise the classification of the criteria established for the type of beneficiaries regarding health education and the number of visits projected by each family group in order to guarantee compliance with the objective of the Community Health Program.
  • Coordinate with the Communications Officer to contribute and maintain communication networks with migrants and support the exchange of information.
  • Plan and execute other community health activities as required by the Health Program Coordinator.
  • Periodically review the information referenced in RR in accordance with the information provided to each volunteer, preventing duplication of information.
  • Generate the QR code and periodically enter into the databases information corresponding to pregnant women, women of childbearing age and children under 5 years of age to receive care at the contracted IPS.
  • Coordinate with the Health Systems Strengthening Supervisor the monitoring of special cases and patients diagnosed with COVID 19.
  • Participate in strategies to improve adherence to the community health program

Manage referrals and be a patient advocate

  • Facilitate communication between programs to guarantee care in health centers for consultations or treatment, and accompany them when necessary and ensure that quality care is received.
  • Act as an advocate for patients to ensure migrants receive the care they need, especially in emergency situations.
  • Carry out quality control checks at all levels of the services offered, guaranteeing timeliness, relevance and effectiveness (community referrals, APS,).
  • Guarantee and follow up on the beneficiary information registration system, carried out by volunteers so that it is pertinent, timely and accurate.
  • Guide volunteers in the process of insurance, regularization and care routes

Strengthening of the health system

  • Provide technical support to the volunteer team to ensure that all objectives are carried out from a public health and primary health care approach
  • Strengthen health care routes from the timely and accurate identification of the needs of the population
  • Support the public health surveillance system through timely notification of risk situations in the community

Coordination and management

  • Promote strong coordination and collaboration between the team and the project.
  • Promote the participation of diverse people in the program such as population of different ethnic groups, sexual orientation, religion.
  • Complete all forms and templates for monitoring, evaluation and management, including monthly reports and other reports requested.
  • Review Red Rose records on a daily basis to ensure delivery of items to the beneficiary population.
  • Communicate timely and directly to the Coordinator of the Health Program and the Coordinator of Community Health the development of scheduled activities and unforeseen events.
  • Schedule, together with the community health coordinator, the postpartum visits to the pregnant women who are followed up.
  • Carry out the monthly work plan of the volunteers and that includes direct supervision of them in the different visits made to the families.
  • Attend coordination meetings as needed, including with government, NGO and UN agencies, and community leaders.
  • Identify and foster opportunities to enhance the impact of the project through partnerships and collaborations with other local stakeholders.
  • Ensure the protection of program participants and employees by promoting an environment of awareness and compliance with the provisions of the code of conduct of policies for the protection of medical teams against sexual exploitation and abuse, protection of children and vulnerable adults and workplace policies free of harassment.
  • Support the protection of program participants and employees by promoting an environment of awareness and upholding the provisions of the code of conduct for the Protection of Medical Teams from Sexual Exploitation and Abuse, the protection of Children and Vulnerable Adults and the harassment-free workplace policies. Holders will remain alert and responsive to any child and adult protection risks, acquire relevant knowledge and skills that will enable them to promote sound protection practices, understand the child and adult protection policy and procedures, and behave in a manner consistent with the Protection.

Education and Certifications

Required

  • Nursing Assistant, last semesters of Professional Nursing, Social Sciences or related careers in Public Health, current Rethus certification certified by educational institutions endorsed by the Ministry of Education.
  • Related diplomas in community health, health education or health communication.

Experience

  • Minimum three years of experience related to community work
  • Work experience with NGOs working on projects related to the public health sector and community health programs.
  • Experience and knowledge of the National Health System
  • Previous work with migrants or refugees.
