Médecins Sans Frontières, an international humanitarian medical association created in 1971, provides medical assistance to populations whose lives are threatened: mainly in the event of armed conflicts, but also epidemics, pandemics, natural disasters or even exclusion of care.
Diabetes specialist responsible for evaluating and contributing to improving diabetes management in the MSF projects in Carnot and Bria, in the Central African Republic.
Context
In 2009, MSF opened an emergency project in response to an extremely severe nutritional crisis in CAR. In 2010, following a retrospective mortality survey, MSF resumed HIV/TB care at the Carnot district hospital. Over the years, the project has focused on improving the quality of care, in particular through the establishment of a follow-up clinic and the integration of adult outpatient consultations.
MSF provides long-term monitoring of adults and children with HIV and non-communicable diseases (NCDs), including diabetes, epilepsy, asthma and kidney failure. This strategy made it possible to detect 1572 cases of NCD, of which 82 cases of type 1 diabetes.
Since 2024, the project “A Pathway to Patient Empowerment by Transforming Diabetes Care in MSF” aims to improve the standard of care for people living with type 1 diabetes (T1D) in Carnot.
The new care package emphasizes patient empowerment by improving access to insulin self-injection and home blood glucose self-monitoring.
Staff have received training dedicated to the clinical management of T1D, therapeutic education is now more formalized, and patients are empowered. Instead of coming to the hospital twice a day for a biphasic insulin injection, patients now receive a clay jar for home storage, a blood glucose meter, and a month’s worth of insulin pens for self- administration.
The next phase consists of transition patients from a regimen based on human NPH or biphasic insulin to a basal-bolus scheme with analogous insulins.
Mission
Support the introduction of new insulin analogues and accompany the transition to the basal-bolus scheme. Support therapeutic education activities, particularly for learning the new basal bolus insulin regimen.
Main responsibilities
Provide refresher training on type 1 and 2 diabetes for clinical staff (outpatient consultations and hospitalization). Provide technical support during consultations and hospitalization activities.
Examine the pathway between hospitalization (IPD) and outpatient consultations (OPD) of diabetic patients, and evaluate transfer mechanisms.
Carry out patient care reviews.
Train and support staff on consultation skills for the management of diabetes (including type 2 diabetes excluding insulin), including: collection of history, analysis of self-monitoring glycemic results, screening for complications, adaptation of doses of analogous insulin according to the results.
Evaluate the relevance of treatments prescribed to patients (in particular the use of insulin).
Train and support Clinic staff in the adjustment of insulin according to the MSF Guidelines, particularly during the transition of patients to the basal-bolus regimen (Human NPH or biphasic insulin to the basal bolus analogue insulin regimen).
Profile sought
- Doctor, specialized(e) or with strong experience in diabetes and/or endocrinology.
- Solid clinical experience in the management of diabetes in adults (consultation and hospitalization). Experience in pediatrics is an asset.
- Minimum 3 years of clinical experience.
- Last clinical experience less than a year old.
- Training in tropical medicine or experience in a resource-limited setting desired.
- Experience in training, supervision and capacity building.
- Good command of French (oral and written).
- Flexibility and ability to work in a team.
Employment conditions
Status: 2-month full-time fixed-term contract, based in Carnot, Central African Republic. Field position, level 9 remuneration according to the MSF field salary scale and based on seniority and relevant experience for this position. Health insurance, transport, accommodation and subsistence costs (perdiem) covered by MSF. Leave according to MSF break policy.
