The humanitarian context in Mali remains marked by pre-existing shocks (conflict/insecurity, droughts, floods and epidemics). Mali currently has twenty-two million affected people, of whom 7.5 million are in need of humanitarian assistance, among whom 51.5% are women, 54.2% children, 3.38% elderly and 15% of disabled people. More than a third of these vulnerable people are located in 21 of the 75 districts, including 13 districts in the Gao, Ménaka and Kidal regions. Since March 2020, the coronavirus pandemic has added to these shocks, increasing the vulnerability of the population. As of May 5, 2023, COVID was declared no longer a public health emergency of international concern, but it nevertheless remains a public health concern and calls for a move toward integrating COVID vaccination into routine.
Vaccination is the safest and most effective way to prevent the spread of vaccine-preventable diseases, including COVID-19. According to the vision of the Immunization Agenda 2030 (IA2030), which stipulates a world in which every individual, wherever they are, whatever their age, fully benefits from vaccines for their health and well-being . And Gavi’s 5.0 strategy advocates leaving no one behind and reducing the number of zero-dose and vaccinated children by 50% by 2030. Mali is one of the countries in the sub-region where vaccination coverage of children under one year old remains stationary. Indeed, according to WHO/Unicef ​​2022 estimates, DTP3 vaccination coverage remained at 77% in Penta 3 in 2022 as in 2021 after a drop to 70% in 2020 due to COVID. These WHO-UNICEF estimates consider that approximately more than 150,000 zero-dose children have been identified and registered in Mali in 2022.
According to the 2018 EDSM, disparities in vaccination coverage were noted between regions and even within regions. These disparities are also observed between rural and urban areas, different socio-economic quintiles, and different levels of education of mothers.
The Bamako District had the best Penta3 coverage with 82%, followed by the regions of Ségou (76%), Mopti (71%), Koulikoro (70%), Sikasso (69%) and Kayes (69%). The northern regions had the lowest coverage rates: Timbuktu (51%), Gao (39%), Kidal 46.8.).
In urban areas (Bamako), coverage was 82% among children aged 12-23 months in Penta3 compared to 68% of children residing in rural areas. The mother’s education, as in the past, remains a determining factor in unequal access to vaccination. Indeed, vaccination coverage is 82% among children born to mothers/caregivers with secondary education and more compared to 68% among those whose mothers have no education.

Purpose of the Position

Under the supervision of the Health Specialist from the UNICEF Gao office, the staff will have the mission
of providing technical support in the coordination of immunization activities, epidemiological surveillance, and EPI logistics in the regions of Gao, Menaka and Kidal .

Main Tasks and Responsibilities:

  • Participate in the annual update of district micro plans and the selection/renewal of pro-equity interventions to be implemented in the districts,
  • Support the establishment and monitoring, using a participatory approach with DS partners in the Gao, Ménaka and Kidal regions, of an annual timeline of equity action plan activities.
  • Provide technical support for the development/updating of the social mapping of vaccination stakeholders both at the district level and at the health area level;
  • Participate on the one hand in the establishment, monitoring of the operation and reporting of the activities of community vaccination support platforms and on the other hand ensure the regular holding of meetings to monitor the accountability framework in the districts of the regions of Gao, Ménaka and Kidal.
  • Provide technical support for the programming/implementation and reporting of vaccination strategies: the Fixed Center, the Advanced Strategy and Mobile Strategy, including the programming of vaccination strategies in conflict situations (periodic intensification of vaccination activities of routine or PIRI, vaccination campaigns or SIA).
  • Participate in the analysis of the security situation and identify strategies (corridors) to achieve vaccination targets.
  • Participate in the development of targeted formative supervision plans from the region to the DS and from the DS to the CSCom, based on an analysis of the strengths and weaknesses of the program.
  • Encourage within the regional teams the monthly analysis of the vaccination coverage results of the DS, including the triangulation between vaccinated children and the quantities of vaccines used, accompanied by action points to improve the insufficiencies.
  • Monitor the monthly situation of vaccine stocks by DS and by each antigen and support the preparation of the monthly regional SMT report.
  • Take weekly stock of the epidemiological situation of Vaccine-Preventable Diseases in the regions with a view to alerting and anticipating participation in possible responses.
  • Provide technical support for the analysis/validation of DIHS2 data and the preparation of presentations during quarterly review sessions of EPI data and establishment of the quarterly EPI bulletin.
  • Contribute to the establishment of a monitoring and monitoring framework for interventions aimed at difficult-to-access communities and zero doses.
  • Monitor biannually the dashboards and bottlenecks of vaccination coverage in priority districts during the mid-term evaluation workshops of the microplans
  • Provide technical support for the development of social mapping of potential vaccination stakeholders in conflict zones both at the district level and at the health area level;
  • Ensure coordination of NGOs involved in vaccination in conflict zones (center and north).
  • Develop the supervision plan for the team of national consultants under his responsibility as well as the UNICEF staff involved in vaccination
  • Ensure technical supervision of national consultants in charge of vaccine management in targeted regions
  • Support the establishment of tools for the integration of COVID-19 vaccination into routine EPI.
  • Provide guidance and technical support to the regional working group for the introduction of the COVID-19 vaccine into routine and the targeting and prioritization of at-risk groups
  • Support the development of the vaccine deployment plan in the regions within its scope of action.

Skills and qualifications 

A Doctor of Medicine degree completed by a Master in Public Health (Master 2)
• At least 2 years of experience in the field of management of vaccination or public health programs;
• Good knowledge of the ACD/Equity strategy and the methods of its implementation
• Good experience in training health workers in EPI management (planning, logistics, communication and immunization in practice)
• Good knowledge of C4D strategies for routine EPI
• Knowledge of English and the local language are desired assets.
