International consultant in charge of evaluating vaccination coverage in Togo
2. Purpose of consultancy
The general objective is to evaluate the level of routine vaccination coverage as well as the functionality of the routine vaccination offer in health structures in Togo.
Specifically, this will involve:
• Determining the proportion of children aged 12 to 35 months fully vaccinated;
• Determine the proportion of children aged 6 to 11 months who are up to date with their vaccinations according to the vaccination schedule;
• Determine the proportions of children aged 12 to 35 months vaccinated with Penta 3, and RR1 and RR2
• Determine the proportion of children aged 12 to 23 months Zero doses using Penta 1 as a reference;
• Determine the proportion of women who gave birth in the last 12 months having received at least two doses of tetanus-diphtheria vaccine (Td);
• Identify the reasons for non-vaccination of children;
• Identify the reasons for non-vaccination with Td;
• Determine the factors favoring vaccination
• Identify the strategies implemented to achieve vaccination targets
3. Background
Since 2002, with the introduction of new approaches, notably the “Reach Each District, ACD” approach, Togo’s EPI has experienced satisfactory development, thanks to the commitment of the State and partners Development.
Faced with changes and emerging diseases that threaten the lives of populations every day, as was the case with the COVID-19 pandemic, the EPI is obliged to continually react to these challenges to preserve the program’s achievements. and find new strategies to cope with these changes.
As recommended by WHO and partners, many countries conduct immunization coverage surveys every 3 to 5 years to monitor the performance of routine immunization services at subnational and national levels, measure the effectiveness of interventions to increase coverage and provide insight into areas of program weakness, for example, by showing the proportion of children on zero doses.
The last vaccination coverage survey dates back to 2012 in Togo. The country carried out a MICS survey in 2017 (which includes vaccination coverage indicators) and a vaccination equity survey in 2017. These surveys are therefore more than 10 years old for the coverage survey and almost 5 years old for the MICS and Equity surveys. It appears necessary to carry out a coverage survey to know the current level of program performance.
In 2022, Togo committed to the development of the new National Vaccination Strategy (SNV) 2023 – 2026 which was validated in March 2023 by the CCIA.
However, the SNV is a dynamic document and Togo intends to update its SNV based on the results of the vaccination coverage survey that will be carried out.
The Covid-19 pandemic has put a strain on health systems in general and the vaccination system in particular. Carrying out a coverage survey in the post-pandemic period will make it possible to evaluate the coverage of all the antigens of the Program, to evaluate the coverage of all the target groups of the Expanded Program on Immunization as well as to analyze the current situation of the Expanded Program vaccination. The results of the survey will make it possible to identify strengths, weaknesses, opportunities and threats, but also to identify the main challenges for the years to come.
Thus, WHO would like to recruit one (1) international consultant expert in immunization coverage assessment to provide specialized skills in data collection, quantitative and qualitative analysis. The consultant will develop an evaluation protocol (quantitative and qualitative), develop evaluation tools, facilitate ethical approval of the evaluation, coordinate the administration of questionnaires, coordinate direct interviews with informants, group discussions , as appropriate, assess the experiences of subnational stakeholders with immunization, analyze the evaluation results and develop evaluation results, including (at a minimum) a report, two case studies and two review documents. information. The consultant will also provide expertise on the vaccination best practice initiative.
The consultant will report directly to the WHO country office representative and will collaborate with a wide range of partners in immunization at the country level including the Ministry of Health.
4. Deliverables
• December 2023: Vaccination coverage survey protocol developed:
• January 2024: Evaluation tools, including for stakeholder commitments and consent, collection and analysis of qualitative data and quantitative and reporting.
• February 2024: Preliminary report of the coverage survey;
• March 2024: Final report, Directory of zero doses and incompletely vaccinated children by district, database and dictionary of variables available.
5. Qualifications, experience, skills and languages
Educational Qualifications:
Essential: A Doctorate degree in medicine or health sciences delivered by an accredited institution; or have a postgraduate degree in epidemiology or public health.
Desirable: A certificate in vaccinology and/or on the new WHO methodology on vaccination coverage survey.
Experience
Essential: Have solid experience of at least 5 years in public health, particularly in the planning and implementation of evaluations of EPI activities. Have conducted a vaccination coverage survey in a country.
Desirable: Have relevant experience of immunization policy in the WHO African region or other experience in EPI program review. Relevant experience within other United Nations agencies, health group partners, recognized humanitarian organizations or non-governmental organizations would be an asset.
