Job Title: Senior Mental Health Clinical Practitioner

Summary: This position provides a variety of specialized services to the homeless chronically mentally ill or other special populations. The employee in this position at a residential program may be required to remain on Organization premises during break and/or meal periods and may be required to stay on Organization premises until their replacement arrives. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Duties and Responsibilities:

·Manages and implements, on a weekly basis, grand rounds to ensure in-depth problem solving and quality assurance of overall client case management.

·Manages and implements, on a weekly basis, specialized group programming for clients (e.g. medication, self-help and daily living skills development.)

·Reports, in writing, to the Administrator on all grand rounds and in-service schedules. Posts all schedules for staff. Ensures that all staff attends such seminars.

·Coordinates and collaborates with the Mental Health Intake Specialist on potential clients to be referred to therapy. Follow-ups to ensure that services are being rendered to all clients by assigning the clients to a group or individual therapists.

·Monitors, on a weekly basis, all client reviews to ensure that a client case record report is complete and accurate.

·Provides intense advocacy, crisis intervention, counseling and problem solving in a variety of areas for all clients.

·Establishes and maintains a professional and therapeutic relationship within the standards of client profiles, confidentiality laws and the newest clinical research available.

·Assesses, plans and supplements the client’s level of functioning.

·Develops, implements, oversees and follows-up with client treatment plans to ensure that realistic goals and deadlines of accomplishments are being met.

·Maintains documentation of client’s individual and group treatment.

·Maintains the volume of work designated by project standards and utilizes the mental health theory and concepts integrating in clinical practice. On an ongoing basis, assimilates the knowledge of resources available to implement the clinical concepts.

·Assumes all clinical responsibility for clients.

·Manages and participates in all project meetings and maintains the minutes of meetings on a rotating basis.

·Presents clinical case reviews.

·Develops and maintains relationships with outside providers, case managers and hospitals.

·Coordinates and attends clinical rounds of all outside client resources.

·Attends a minimum of 15 educational trainings per fiscal year.

·Maintains medications as specified by the Psychiatrist and keeps legible and accurate records of all medication issues.

·Consults with the Psychiatrist regarding treatment reviews.

·Manages and signs for Medicaid, Medicare and other third party liability billings.

·Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies and Unit guidelines. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. ·Education and/or Experience: Master’s degree (M.A.) in clinical social work, counseling or clinical psychology; and three years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy.

·Certificates, Licenses, Registrations: Licensed in Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC).

·Other Qualifications: A valid driver’s license may be required.
